If you care to sponsor a golf-hole, please complete the form below. Also, we have been directed that this years’ sponsorship payments must be received before the event. The Annual Conference Golf Scramble will be held at the Canaan Valley Golf Course, Friday July 13. We request that you arrive at 7:30 a.m. to receive instructions prior to Tee Time at 9:00 a.m. when there will be a Shotgun start.  Golf Hole Sponsorship My Company would like to be a “hole sponsor” for the Golf Scramble at the rate of $200 per hole. All hole sponsors will be recognized during the golf outing with signage at the tee boxes and during the Annual Meeting.
