The Dr. Meryl J. Efron Memorial Lecture Prosthodontics—A 40-Year Retrospective: Where We Have Come from and Where We are Headed Featured Clinician: Louis F. DeSantis, D.D.S. Former Northeast Regional Director, American College of Prosthodontists; Adjunct Clinical Associate Professor, Advanced Education Program in Prosthodontics, New York University College of Dentistry Revolution or evolution? The last 40 years have witnessed tremendous growth and change in how we approach our treatment protocols. From advances in adhesive dentistry, implant dentistry, digital dentistry and evidence-based dentistry, the last 40 years have seen the greatest growth in both didactic and treatment workflows. With rapidly emerging technologies, how are we to focus on the future of our profession: utilizing AI to enhance our treatments or a return to the fundamentals? Prosthodontics (restorative dentistry) underwent a revolution in 1983 and again in 1998 with the publication of three seminal research articles: • Osseointegration and its experimental background. Brånemark, PI. J Prosthet Dent. 1983 Sep;50(3):399-410. doi: 10.1016/s0022-3913(83)80101-2. • Etched porcelain facial veneers: a new treatment modality based on scientific and clinical evidence. Calamia, J.R. J Dent. 1983 Sep-Oct;53(6):255-9. In 1989 and 1999, digital impression and computerized CAD-CAM technologies were the second revolution. • Dental CAD/CAM technologies: the optical impression (I). [Article in English, German]. Pfeiffer, J. PMID: 11351456 was published in 1998 with part II published in 1999. Where we have evolved over the last 40 years as a profession came from inspirational, seminal research, along with vision. Since these early publications, our knowledge, techniques and work flows have changed dramatically. This presentation will look at the evolution of prosthodontic therapies and discuss, “where we came from and where we are headed.” The future is now. Upon successful completion of this lecture, participants will have an understanding of what truly is “the golden age of dentistry,”: implantology, digital dentistry, aesthetic dentistry and dental mimetics. All these separate areas are interrelated and changing the way we look at dental issues and use best evidence to treat our patients. An advantage of ceramic restorations is the ability to bond them to tooth structure. This allows for a more conservative tooth preparation. However, cement selection and technique is critical to success and one material and procedure will be presented that can be used with slight variation on all ceramic restorations. Upon successful completion of this lecture, attendees will have an understanding of the advantages and disadvantages of the various ceramic materials currently available. This will aid in selecting the right material for different clinical situations. There will also be a brief review of cementation of ceramic restorations.
