New ADA Dental Claim Form Coming in January 2024 by Alyson K. Buchalter, D.M.D. The 2024 American Dental Association (ADA) Dental Claim Form goes into effect January 1, 2024. The ADA, (@americandentalassociation), has produced a new video regarding the upcoming changes to the 2024 ADA Dental Claim Form. The changes include indicating if claims are delivered by a Locum Tenens dentist, the date of the last scaling and root planing (box 29a) to expedite timely claim adjudication, and a space for the identification number selfassigned by third-party payers known as the payer ID (box 3D and 11D). A Locum Tenens is the term for a dentist who is standing in for another who is away for a short time for any reason including staffing shortage, leaves of absence, short-term disability, etc. There is a check box under the treating dentist’s signature field to indicate if the treating dentist is a Locum Tenens.
