The New York State Dental Association (NYSDA) makes up the entire second trustee district constituent of the ADA. Our local component, the Second District Dental Society (SDDS), is one of 13 components that make up NYSDA. This year the ADA convened its HOD in Orlando from Saturday, Oct. 7 until Tuesday, Oct. 10. SDDS, as part of the larger NYSDA caucus, was represented by Drs. Alyson Buchalter, Tricia Quartey-Sagaille and Paul Teplitsky as our delegates and by Drs. John Demas, Raymond Flagiello and Mitchell Mindlin as our alternate delegates. At this year’s meeting, representatives were tasked with the analysis and deliberation of a total of 79 resolutions that would be put before the House. Seventy-nine resolutions to be analyzed and deliberated within but two sessions of the House held over the course of just four days? The reality is that the process, at least for the NYSDA delegation, had begun long before the actual governance meeting. Buchalter and Demas served on Dental Benefits, Practice and Related Matters. In mid-August the expanded caucus, (caucus chairs and NYSDA board of trustees) met via Zoom. During the next two weeks, individual study group meetings were held during which every delegate and alternate delegate from NYSDA was given a bit of homework. Each was assigned a handful of those resolutions and asked to become the authority on them. In the first week of September, the second trustee district pre-caucus meeting convened via Zoom. The NYSDA Board, component delegates and alternate delegates and NYSDA staff met for four plus hours during which protocol and rules were reviewed and discussions were held regarding the two resolutions that had been submitted by NYSDA to the ADA board of trustees (ADA BOT). Study Groups (Membership, Education, Legislative, Benefits and Budget) broke out into smaller meetings within the meeting and the previously assigned “homework” was reviewed. After the study groups completed their discussions, the entire caucus reconvened to review the findings and recommendations of the groups. Once in Orlando, on the day before the first session of the House, the expanded caucus (caucus chairs, NYSDA board of trustees and staff) met, yet again, to review what had previously been discussed as well as to consider new resolutions that had been submitted too late to have been reviewed by the ADA BOT. The morning of the first official day of the meeting saw our caucus hard at work, with the study groups yet again hashing out the details of the resolutions before them, gathering more information from subject matter experts from the ADA or from our colleagues from other trustee districts who were the makers of some of the resolutions. The afternoon welcomed in the first meeting of the HOD. As with all such meetings, a bit of time at the first session is spent on speeches, recognition of service by individuals and introduction of special guests and dignitaries (and possibly more speeches). This session was no different. The second day of the meeting, Sunday, saw virtually six hours of non-stop testimony for or against every resolution brought before the House in the form of four Reference Committee Hearings. Reference Committee hearings are an extremely important part of our meetings, as it is during these sessions that we learn of arguments (pro or con) that we might not have considered, hear of unintended consequences of certain resolutions or get clarification of intent. Monday saw our delegation (from 8 a.m. until 3 p.m.), again by study groups, putting the final touches on our recommendations, with numerous interruptions in the form of presentations to our group by the candidates for the office of ADA president-elect and second vice president (the voting to take place at the final session of the House). Before adjourning, each of the study group chairs had presented their committee recommendations to the entirety of the caucus with lively discussion throughout. The recommendations were ultimately compiled by staff and printed for distribution to our delegation so that each and every one of the representatives would attend the final sessions as well informed as they could possibly be. The voting for the elective offices commenced at 7:30 a.m. on the final day of the meeting. Dr. Brett Kessler from Colorado was elected ADA president- elect and Dr. Edwin del ValleSepulveda from Puerto Rico was elected ADA second vice president. Consideration of all the resolutions and all proposed amendments and substitutions were debated and voted upon during the final two sessions of the House. It was also officially announced that NYSDA’s Dr. Maria Maranga (of Suffolk County Dental Society) will be seeking the position of ADA president-elect in 2024 and Dr. Viren Jhaveri (of Queens County Dental Society) will be seeking the position of ADA second vice president. Don’t Let a Short-Term Illness Knock You Out by Stuart L. Segelnick, D.D.S., M.S., C.D.E., SDDS Bulletin Editor Most dentists have experienced some form of illness during their careers. There was even a time when working with a cold was considered standard dental practice procedure for many dentists in solo practice. Today we better understand the ethical dilemma of working while sick, which can bring an increased risk to the health of dentists’ employees and patients and affect the dentist’s decision-making capacity. A sickness that has caused the dentist to stay at home and not function could have a devastating effect on the practice. There are many different types of aid the dentist may be entitled to during these trying times such as disability insurance and social security disability. However, if the illness lasts for a short period of under a few weeks, then there is no vehicle for recouping the economic loss. Looking into mitigating the financial and collateral damage of a short-term work interruption before the incident occurs may be a prudent course of action. A few months ago, I personally had to deal with a health problem that closed my office. My gallbladder was the culprit. The gallbladder is used by our bodies to store bile and release it when fatty food is being digested (see https:// www.ncbi.nlm.nih.gov/books/ NBK279386/ and https://newsin health.nih.gov/2015/02/galled- gallbladder). Otherwise, the gallbladder is not an organ that is necessary for survival because in its absence the liver, which produces bile, will continue to do so at a slow rate, emptying into the small intestines. My gallbladder was filled with stones which I saw clearly on a sonogram and special MRI called a magnetic resonance cholangiopancreatography (MRCP) that the surgeon ordered. It had caused a few episodes of common bile duct obstruction leading to severe crushing pain in my right chest and back — severe agony that Photo of the author’s gallstones on top of an inverted 8-ounce plastic cup after nail polish application for long-term preservation had me doubled up and vomiting all night long. After the last attack, when my gallbladder became infected and almost had me hospitalized, I decided it was time to explore surgical options of removal (cholecystectomy), especially when my physician said it could become life threatening. Since it was a planned surgery, it gave me time to plan for my absence. My surgeon had informed me that I could most likely return to work in five days, although most of my own online research pointed to two weeks. The patients were cancelled and the staff were coached on what to do while I was away. They would manage the office while I was gone and call me if there was anything that couldn’t wait. There are many consequences to being unable to practice for even a small amount of time. One of the first things you should do is have an emergency contingency plan. A very helpful website, the American Association of Disabled Dentists (AADD) has a well thought out “Emergency Practice Continuity Care Plan,” which you can find at https:// disableddentists.org/2016/06/24/ activate-your-practice-continuity- plan. I contacted one of the founders of the AADD, Dr. Ronald Marsh, who started the organization after having a stroke and finding the American Dental Association was not helpful in his plight. Dr. Marsh and three other people who have experienced terrible health issues are available to support and counsel any dentist who has questions on how to proceed during the trying times of disability. Dr. Marsh advised that when purchasing your disability insurance, you make sure there is a short-term disability clause, which could have helped me in my situation. He also counseled, “When purchasing disability insurance, do it NOW; never delay, and get the best policy you can afford. This is NOT the time to be cheap. It could be the best investment you ever make. Ensure it has an “own occupation clause,” and be open to buying a second policy. And yes, you can have more than one policy.” He also stressed paying for your policy with post-tax dollars, which is a huge benefit, otherwise your benefits will be taxed. Dr. Marsh recommended buying a disability policy with a cost- of-living rider along with the option to increase purchased benefits as your practice grows and you have a higher income. If there are lifetime benefits available, Dr. Marsh recommended purchasing them too; although costly, he said, “It is well worth it.” As a second backup if you are not in a group practice, Dr. Marsh said to make sure you get the practice an overhead rider, which will cover your rent, utility 4 SDDS BULLETIN NOVEMBER/DECEMBER 2023 — VOLUME 40 ◊ NUMBER 6

bills, insurance premiums for the practice and even your membership dues. Dr. Marsh stressed that most disability policy benefits do not kick in for 90 days. Hopefully you’ll get better before then; however, if you don’t, make sure while receiving your medical care to maintain “rock solid documentation that you are disabled.” He also said to make sure you STOP working because if you don’t, it will weaken your claim. Always make sure you read your policy and then file a claim. Dr. Marsh suggests filing the claim yourself and if you are denied benefits, then it’s time to hire an attorney and file an appeal. During my recovery, I called my disability insurance carrier who said that I had their premier plan, which made me happy until he continued that the policy would only begin paying out benefits after 60 days of disability. Since I was planning to be back to work much quicker, my disability plan would not cover my short-term absence. The agent suggested most dentists put away a couple of months of money, literally self-insuring these short events. He then compared the time away to taking a vacation. Needless to say, I was very upset to hear this analogy because my experience was so incongruous to joyful times. Being prepared for short- and longer-term illnesses at the office is extremely important to a dentist’s practice survival. Having a well-crafted emergency contingency plan will also help alleviate pressure from the stress of coping with a sudden illness and avoid additional mental anguish. As I continue to recuperate, I am reminded that without your health, everyone and everything else you love and treasure may never be available. So please, take care of yourself! New ADA Dental Claim Form Coming in January 2024 by Alyson K. Buchalter, D.M.D. The 2024 American Dental Association (ADA) Dental Claim Form goes into effect January 1, 2024. The ADA, (@americandentalassociation), has produced a new video regarding the upcoming changes to the 2024 ADA Dental Claim Form. The changes include indicating if claims are delivered by a Locum Tenens dentist, the date of the last scaling and root planing (box 29a) to expedite timely claim adjudication, and a space for the identification number selfassigned by third-party payers known as the payer ID (box 3D and 11D). A Locum Tenens is the term for a dentist who is standing in for another who is away for a short time for any reason including staffing shortage, leaves of absence, short-term disability, etc. There is a check box under the treating dentist’s signature field to indicate if the treating dentist is a Locum Tenens. SDDS PARTICIPATES IN BROOKLYN OUTREACH EVENT by Esther Nelson, D.D.S. On Saturday, Aug. 26, members of Second District Dental Society’s Oral Health Committee participated in an incredible back-to-school event in Downtown Brooklyn. The Second District Dental Society (SDDS), along with Colgate Bright Smiles, Bright Futures (BSBF) and Bethel Baptist Church, formed a successful trio collaboration to provide sufficient resources to the growing community. Many children were present with their parents in attendance. This became a great opportunity to recruit children under the age of 12 for dental screening and to give out free supplies. Two young Colgate BSBF coordinators — Lakeshia and Matthew — were extremely helpful in providing volunteers with the proper protocol for dental screenings conducted inside the BSBF mobile dental van, as well as in educating the children with the props presented on the tabletop outside. SDDS is thankful to the following participating dentists: Drs. Lauren Smith, Natasha Patel, Michelle Shamaev, Esther Nelson and Sophia Scantlebury. Dr. Scantlebury is a well-known orthodontist in Brooklyn and her expertise was needed for children in need of orthodontic treatment. SDDS community outreach looks forward to future collaborations with the Colgate Bright Smiles, Bright Futures program. The last 40 years have witnessed tremendous growth and change in how we approach our treatment protocols. From advances in adhesive dentistry, implant dentistry, digital dentistry and evidence-based dentistry, the last 40 years have seen the greatest growth in both didactic and treatment workflows. With rapidly emerging technologies, how are we to focus on the future of our profession: utilizing AI to enhance our treatments or a return to the fundamentals? Prosthodontics (restorative dentistry) underwent a revolution in 1983 and again in 1998 with the publication of three seminal research articles: • Osseointegration and its experimental background. Where we have evolved over the last 40 years as a profession came from inspirational, seminal research, along with vision. Since these early publications, our knowledge, techniques and work flows have changed dramatically. This presentation will look at the evolution of prosthodontic therapies and discuss, “where we came from and where we are headed.” The future is now. Upon successful completion of this lecture, participants will have an understanding of what truly is “the golden age of dentistry,”: implantology, digital dentistry, aesthetic dentistry and dental mimetics.
